2010/2011 Developmental (LSYSA) Registration form: This is the primary form used to enroll your child(ren) AGED 7 AND OLDER in Cainhoy Athletic Developmental (LSYSA) soccer program. Each family will also need to additionally complete a medical release form and a copy of birth certificate. |

Medical release: USYSA and SCYSA medical release form to be completed by every player at time of registration. |

Sponsorship Opportunities: A document describing corporate advertising, support, and player scholarships. Please help our club by donating funds to help support those most in need. |
